Shiitake's head is a quest item for the paranormalist quest "Earning Respect." It is a recipe in botania discovered after contacting the Elves. It can be made by placing a skeleton skull, golden apple, a nametag (with "Shiitake" written on it), pixie dust, and a prismarine shard onto a runic altar.
